Non-Identifying Background Information

 

We at Children’s Aid Society of Utah are pleased to be able to provide nonidentifying information to the adoptive children placed through our agency and to the birth parents of those childen who were placed. Such information includes any physical, medical, educational, occupational, religious, and social information that is available on birth parents and adoptees.

In order to receive such information, please fill out the correct form and return it to the agency along with a $50 fee. Your questions and comments are welcome.

Please be advised that the agency receives many requests of this nature, therefore you may not receive your information for up to several months. Your request will be answered as soon as possible in the order that it is received.

You are welcome to write a letter with or without identifying information to be retained in your file. This letter will be given to the birth parent(s) or the adoptee in the event we have contact with them.
